Parent’s Night Out

This STEM education program is not a licensed child-care facility or daycare, and is not regulated or inspected by the State of Texas. STEM Tech provides educational enrichment opportunities only and does not offer child-care services. Acknowledgement (Required): I understand that STEM Tech is not licensed or regulated by the State of Texas as a child-care facility.

In case of an emergency, STEM Tech staff will attempt to contact the parent or guardian immediately. If contact cannot be made, this form authorizes STEM Tech staff to seek emergency medical care as deemed necessary. I authorize STEM Tech staff to administer basic first aid and, if necessary, seek emergency medical care for my child. Acknowledgement (Required): I understand that STEM Tech is not licensed or regulated by the State of Texas as a child-care facility.

Media Release: I hereby grant permission for my child to be photographed or recorded during STEM Tech activities. I understand these images may be used in promotional materials, social media posts, and advertising. No compensation is provided. I may revoke consent at any time in writing. I authorize STEM Tech staff to administer basic first aid and, if necessary, seek emergency medical care for my child. I understand that participation in STEM Tech programs involves hands-on STEM activities that may include tools, robotics equipment, electronics, computers, and small building components. I acknowledge the risks of minor injury such as cuts, scrapes, pinches, or allergic reactions. I agree to release and hold harmless STEM Tech LLC, its owners, instructors, and staff from any claims of liability for injuries or damages occurring during the program, except in cases of proven negligence.

Students are expected to behave respectfully toward peers, staff, and equipment. STEM Tech reserves the right to remove any student from the program for repeated disruptive or unsafe behavior. Refunds may not be issued in such cases.

Things come up and we understand that. We also try and make sure that we plan for your child ahead of the event. Our refund policy attempt to accommodate various situation as follows: Advance Cancellation (24 to 48 hours prior): Full refund or account credit minus a small administrative fee, as staffing and pizza/craft counts are not yet finalized. Late Cancellation (Under 24 hours / Day-of): No cash refund; instead, issue a facility credit or assess a late cancellation fee (e.g., $15–$25) to cover purchased meals and reserved staff ratios. No-Shows: Forfeit the registration fee entirely with no credit issued, since a spot was held and pre-paid supplies/food went unused.
